Initial Comments:
Based on the findings of an offsite revisit completed December 30, 2024, VNA Hospice, Western PA had corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a Medicare recertification and state-license survey completed September 27, 2024.
